A 42 year old female patient from Bangalore approached us with a complaint of shaking tooth supported bridge which was done 9 years back in a private clinic in Bangalore and few root canal treated teeth without crowns.
ON CLINICAL EXAMINATION
Tooth supported bridge was mobile, few dislodged crowns of root canal treated teeth. Digital documentation was done which shows left lateral gummy smile.
RADIOGRAPHIC EXAMINATION
Radiographs revealed there was a fracture present between the crown and the roots of tooth supported crowns, all the root canal treated teeth sounded good and the bone quality was good for dental implant placement.
TREATMENT PLAN DISCUSSION
After complete clinical and radiographic examination, treatment plan was discussed with patient to remove the tooth supported bridge and extraction of 1st infected teeth alone and replacing it with zirconium dental implants under local anesthesia(STA-painless injection) followed by digital scanning and giving tooth and implant supported crowns the very next day. And to follow postoperative followup instructions.

- During the initial few months of treatment, patient was comfortable with the tooth supported bridge, then after she started to notice that the bridge was shaking and food getting stucked between the bridge and the gum which gave pain both in her gums and teeth as well.
- As days goes on the pain got worsen and the gums surrounding the teeth was infected. Also the patient complaints of gummy smile on left side because of which the patient was hiding her smile for years. Patient wanted to correct her smile and wants a fixed permanent teeth with dental implants in 3 days at affordable cost and finally approached us.

Implant supported CAD CAM milled pmma bridge was fixed over the implants and tooth as a connected bridge which finally corrected her gummy smile.
Patient was very happy that her gummy smile has disappeared and she was able to chew, speak, brush without any pain in her teeth. Patient was enjoying her foods and good smile without hiding her teeth.
Finally the patient was asked to report us after 4-6 months of healing period to change her provisional crowns to permanent zirconium crowns
