Fractured root canal treated teeth replacement with dental implants in a day in chennai, India

                             A young male patient aged 26 years from mumbai visited our clinic with a complaint of broken root canal treated teeth. On questioning about the past dental history, we came to know that the tooth was root canal treated 12 years back in other outside private clinic and tooth supported zirconium crown were given. Now the tooth structure has fractured with the zirconium crown and he was having mild pain in that tooth. Patient wants a fixed permanent solution and to regain his smile back. Also patient told that he is leaving to mumbai the very next day and wants to finish his treatment in a couple of days.

                             CLINICAL EXAMINATION

                      The tooth was tender on percussion and the surrounding gums were painful on palpation with broken root fragments

RADIOGRAPHS-  OPG and CBCT taken revealed a perapical cyst present around the apex of the tooth and we explained the patient that root canal treated tooth was reinfected with pus formation and this was the reason for his pain



                                    TREATMENT PLAN

       After complete examination, treatment plan was discussed with the patient to extract his broken root canal treated teeth followed by replacemnt with basal dental implants under local anesthesia in a day

              DENTAL IMPLANT PLACEMENT PROCEDURE

          After getting written consent from the patient,under local anesthesia the tooth was extracted, after extraction gentle irrigation and curretage was done to remove the cyst followed by irrigation again. Finally one basal dental implant was placed and suturing was done to approximate the gums.

                       

Followed by this, 3D SCANNING using imedit500 intra oral scanner (digital impression) was taken and the prosthesis was delivered on the same day.



FINAL OUTCOME 

After taking the digital impression , prostheis(PMMA crown)  was delivered on the same day. 


 Patient was happy with the final outcome, and we informed the patient to report us after 4 to 6 months to change his provisional crowns to permanent zirconium crowns when he return backs to chennai

Shaking teeth replacement with dental implants in chennai, India

                  A 44 year old male patient from andhra pradesh approached us with a complaint of shaking lower front teeth with bleeding and swollen gums. The patient stated that his lower front teeth started to shake a year back, which over time led to severe mobility. He was more concerned about his crowding of lower front teeth and told he is having difficulty in brushing there and thought  that might be the reason for his loose gums. As a result of mobility and gum infection, patient felt difficulty in biting food and bleeding gums while brushing his teeth. So, patient wanted to correct his malalignment as well as to find a solution for his shaking teeth and finally approached us.

                            ON CLINICAL EXAMINATION-

           Patients lower front teeth appeared to be crowded and malaligned, gum tissue surrounding the relative teeth seemed to have mild inflammation with bleeding on probing and palpation.


                              ON RADIOGRAPHIC EXAMINATION -

              OPG and CBCT  taken shows moderate to generalised bone loss in all teeth except for the corresponding lower front teeth which shows slightly more bone resorption, due to poor maintenance of the patient.

                                      

                                                        TREATMENT PLAN-


             With regards to patient concern on his malaligned and shaking teeth in lower front region and after complete clinical and radiographic examination we planned to extract his lower front teeth and replacing the missing teeth with basal dental implants in a day follwed by teeth fixing. And informed consent was obtained from the patient.
            Then the extraction and  implant placement placement procedure was carrried out under local anesthesia on the same day of consultation and provisional teeth were given on the same day of implant surgery. Patient is informed that these provisional teeth will be replaced by permanent zirconium crowns after a healing period of 4 to 6 months.

Then 3D digital scanning was done fot the delivery of the prosthesis

A natural looking prosthesis was fixed on the implants. The teeth was mimicking the natural teeth and looked very real and was well aligned with the patients natural teeth, enchancing the patient look and smile. The patient was extremely happy with the final outcome.

Dental implants for missing back teeth in chennai, India

 A 52 year old patient from Singapore approached us with a complaint of missing back teeth since 10 years and wants a fixed solution for his missing teeth in a couple of days. Patient gives a history of wearing removable denture since 2 years and it was broken 2 days back. Patient felt odd and distressed with his teeth, and was hiding his smile since 10 years and finally visited us to regain his smile.
        After complete clinical and radiographic [2D(OPG) and 3D(CBCT)] examination, a comprehensive treatment plan was discussed with the patient
              1.to place basal implants in the involved site under local anesthesia on day of consultation
              2. followed by fixing provisional teeth on the same day
              3. after 6 months that is after complete healing , permanent CADCAM zirconium crowns by 3D digital intraoral scanning(imedit500) will be fixed  on the same day

After placing the implants, provisional teeth were fixed, patient was extremely happy with his teeth and returned to singapore and he was recalled after 6 months for his permanent crowns

Finally patient came back to change provisional crowns to permanent crowns, on the same day provisional crowns were removed and 3D digital intra oral scanning was recorded for CAD CAM zirconium crowns

WHY HAVE DENTAL IMPLANTS BECOME A MUST???

THE UNDERSTANDING THAT  UNLOADED BONE CAUSE MORE BONE LOSS has brought an awareness towards replacing missing teeth and at the same time prevent bone loss.

Solutions to the problem of atrophic alveolar bone begin with understanding why unloaded bone goes atrophic. Nobody can exactly depict why alveolar ridge goes atrophic after extraction of teeth. Efforts have been made to elucidate the scientific basis for the observation that the integrity of the skeletal tissue has something to do with mechanical usage. Frost has evolved the concept of the mechanostat (Frost, 1996) which he himself proposed as a new paradigm of understanding the mechanical-usage-dedicated message traffic routes between the skeletal tissue and the circumstances in which it is tuned. He argued the mechano-biologic negative feedback mechanisms would adjust skeletal architecture under the control of a subject’s mechanical usage.


Strain-derived fluid flow transduces the strain information to stimulate bone metabolism.



Skeletal tissue has its loading history by dynamic loading which is remembered as a microstrain.
If the strain is too small value, resorption dominates formation of bone tissue and the bone goes atrophic.

If the strain is adequate, resorption balances formation and the bone maintains its volume.

If the strain is adequately high value, formation at last dominates resorption and the bone goes hypertrophic.

If the strain value is high by the overloading outreaching the range of physiological limitation, the strain is accumulated in the bone tissue causing microdamage leading to fatigue failure.
Finally, if the strain value over the limit of yield strength the bone goes spontaneous fracture.


Due to these reasons immediate loading of dental implants will result in functional replacement of teeth and prevent any further bone loss.

PRINCIPLES OF ORTHOPAEDIC BONE SURGERY AND A COMPARISON TO DENTAL IMPLANTOLOGY

Although a number of fractures is still today preferably treated by stabilizing the sites with gypsum, surgical procedures and adequate medical devices have been developed during the last three decades, these include:

1. Screws and plates
2. Intra - bony nails
3. Intra - bony blades
4. Basal implants



Dental Implants and the prosthetic construction of the same resemble the same. 

The intraoral situation is special and different from extra- oral applications
1. The mucosal penetration area around dental implants is under constant and vicious bacterial attack.
2. The saliva washes away bacteria as their excretion products and it stabilizes the pH.
3. Jaw bones are not at all prone to osteomyelitic activity

In orthopaedic surgery (knee and hip replacement surgeries) it is taught that immediate loading(using) after plating or screwing will lead to good healing and bone remodelling. The same concept is follwed in immediate loading dental implants. 




Primary implant stability refers to the stability of a dental implant immediately after implantation. Its value is derived from a mechanical engraving  of the titanium screw implant in the patient's bone tissue. High initial stabilization may be an indication for immediate loading with prosthetic reconstruction. When the healing process is complete, the initial mechanical stability is fully replaced by biological stability. The most critical moment for implantation success is the moment very less initial stabilisation, pending sufficient bone reconstruction supporting long-term maintenance of the implant. Usually this occurs during the 3–4 weeks after implantation. If primary stability was not high enough following implantation, the implant's mobility is high and can cause failure.
