Dental implants for failed root canal treated teeth in chennai, India

This is case report of our 32 year old male patient who approached us with a complaint of severe pain in upper front teeth since 1 month. On questioning about the previous dental history, patient gave a history of  2 root canal treated teeth done 8 years back, and tooth supported metal ceramic crown was given to the patient. The other root canal treated tooth was asymptomatic. Patient was not able to speak, that even the tongue pressure over the tooth while speaking and chewing was giving severe pain to the patient, also the patient was not able brush his teeth. Patient itself came to know that the root canal treated teeth was reinfected. then we clearly explained that reinfection in root canal treated teeth is a normal complication and that too it depends on aggressive nature of the bacteria, patient finally understood the current situation of his painful teeth and asked for a permanent solution that would improve his well being.

                               ON CLINICAL EXAMINATION

The tooth was tender on percussion and palpation of surrounding tissues, adjacent tooth was asymptomatic and there was no tender on palpation and percussion.

ON RADIOGRAPHIC EXAMINATION

A periapical pathology indicating pus formation around the apex of the teeth was noticed and mild to moderate bone loss was noticed around the concerned teeth

3D(CBCT) cross section showing the presence of periapical pathology around the apex of the tooth

TREATMENT PLAN

After complete clinical and radiographic analysis,, treatment plan was discussed with the patient, first extraction of painful root canal treated tooth followed by replacemnet with basal dental implants in a day and giving connected tooth supported PMMA crown on the same day of implant surgery.

TREATMENT PROCEDURE

Before starting implant and tooth removal procedure, tooth supported metal ceramic crown was removed followed by extraction and implant placement procedure under local anesthesia.

IMMEDIATE POSTOPERATIVE OPG

Following the implant placement procedure, 3D digital scanning using imedit500 intra oral scanning was done and  tooth and implant  supported PMMA bridge was fixed on the same day of surgery

WHY HAVE DENTAL IMPLANTS BECOME A MUST???

THE UNDERSTANDING THAT  UNLOADED BONE CAUSE MORE BONE LOSS has brought an awareness towards replacing missing teeth and at the same time prevent bone loss.

WHY UNLOADED BONE CAUSE MORE BONE ATROPHY (BONE LOSS)???

MECHANOTRANSDUCTION

Solutions to the problem of atrophic alveolar bone begin with understanding why unloaded bone goes atrophic. Nobody can exactly depict why alveolar ridge goes atrophic after extraction of teeth. Efforts have been made to elucidate the scientific basis for the observation that the integrity of the skeletal tissue has something to do with mechanical usage. Frost has evolved the concept of the mechanostat (Frost, 1996) which he himself proposed as a new paradigm of understanding the mechanical-usage-dedicated message traffic routes between the skeletal tissue and the circumstances in which it is tuned. He argued the mechano-biologic negative feedback mechanisms would adjust skeletal architecture under the control of a subject’s mechanical usage.

Strain-derived fluid flow transduces the strain information to stimulate bone metabolism.

Skeletal tissue has its loading history by dynamic loading which is remembered as a microstrain.
If the strain is too small value, resorption dominates formation of bone tissue and the bone goes atrophic.

If the strain is adequate, resorption balances formation and the bone maintains its volume.

If the strain is adequately high value, formation at last dominates resorption and the bone goes hypertrophic.

If the strain value is high by the overloading outreaching the range of physiological limitation, the strain is accumulated in the bone tissue causing microdamage leading to fatigue failure.
Finally, if the strain value over the limit of yield strength the bone goes spontaneous fracture.

Due to these reasons immediate loading of dental implants will result in functional replacement of teeth and prevent any further bone loss.

PRINCIPLES OF ORTHOPAEDIC BONE SURGERY AND A COMPARISON TO DENTAL IMPLANTOLOGY

Although a number of fractures is still today preferably treated by stabilizing the sites with gypsum, surgical procedures and adequate medical devices have been developed during the last three decades, these include:

1. Screws and plates
2. Intra - bony nails
3. Intra - bony blades
4. Basal implants

Dental Implants and the prosthetic construction of the same resemble the same.

The intraoral situation is special and different from extra- oral applications
1. The mucosal penetration area around dental implants is under constant and vicious bacterial attack.
2. The saliva washes away bacteria as their excretion products and it stabilizes the pH.
3. Jaw bones are not at all prone to osteomyelitic activity

In orthopaedic surgery (knee and hip replacement surgeries) it is taught that immediate loading(using) after plating or screwing will lead to good healing and bone remodelling. The same concept is follwed in immediate loading dental implants.

Primary implant stability refers to the stability of a dental implant immediately after implantation. Its value is derived from a mechanical engraving  of the titanium screw implant in the patient's bone tissue. High initial stabilization may be an indication for immediate loading with prosthetic reconstruction. When the healing process is complete, the initial mechanical stability is fully replaced by biological stability. The most critical moment for implantation success is the moment very less initial stabilisation, pending sufficient bone reconstruction supporting long-term maintenance of the implant. Usually this occurs during the 3–4 weeks after implantation. If primary stability was not high enough following implantation, the implant's mobility is high and can cause failure.

WHY DENTAL IMPLANTS???

Why dental implants???

Esthetics and function are the two most important things that teeth should fulfill.
If the functional aspect of teeth is compromised due to decay, wear and tear, bone loss, gum swelling, it can lead to various health risks.

gradually you might find it difficult to eat or even painful to bring the teeth together in bite. Your teeth might become sensitive to drinking hot or cold. If thiese symptoms go to an advanced stage there might even be tooth loss.

Everyone loves to have a clean white, sparkly and straight teeth that brighten your face and give an attractive smile. Dental implants can help you achieve that.

Before implants

After  dental implants and permanent teeth fixing

A dental implant can be used to replace a single tooth, multiple teeth, or to replace an entire full arch. One of the most significant advantages of dental implants is they are virtually undetectable. No one will notice you have dental implants but they will notice your beautiful smile. You will probably find that you like to smile a lot more too. Why? You will no longer feel embarrassment about your teeth or gums. You'll be proud of your smile and you will be ready to show it off. Another benefit of dental implants is that resolving dental problems will alleviate the physical pain you have suffered from unhealthy teeth. The implants will allow you to eat and drink without experiencing pain and sensitivity as you may have in the past. The dental implant process is a relatively simple and cost-effective procedure, but the changes to your life when complete are priceless. Increased self-confidence can transform your life.

Long term neck and shoulder pain.... Did you check the teeth yet???

We use T scan to check for any bite problem related to natural teeth or prosthesis

Dentistry is a science that is largely affected by its materials and equipment . Lost and damaged teeth
require replacement or rehabilitation . Thanks to the development of differing dental materials and
precision technologies, we are now able to successfully treat clinical conditions that were previously
difficult to predictably improve . Because of the invention and development of dental implants over
the past quarter century, edentulous patients can masticate as if they had their own natural teeth
instead of wearing uncomfortable dentures.

Malocclusion is the result of the body trying to optimize its function in a dysfunctional environment. It can be associated with a number of problems, including crooked teeth, gum problems, the temporomandibular joint (TMJ), and jaw muscles. Teeth, fillings, and crowns may wear, break, or loosen, and teeth may be tender or ache. Receding gums can be exacerbated by a faulty bite. If the jaw is mispositioned, jaw muscles may have to work harder, which can lead to fatigue and or muscle spasms. This in turn can lead to headaches or migraines, eye or sinus pain, and pain in the neck, shoulder, or even back. Malocclusion can be a contributing factor to sleep disordered.

Case 1:
Patient came with the complaint of increased spacing observed recently .

On checking clinically we elicited, interference in the wisdom tooth and premolars. patient gives history of missing teeth since 9 years. gives history of  neck and shoulder pain for a long time which is intermittent.

On intraoral muscle evaluation, patient had severe pain in the muscle attachments on the right side, where there is interference.
At normal occlusion, the teeth are not in contact
Hiting wisdom tooth on normal biting
due to tilt of the wisdom teeth and rotation of premolars as a result of missing teeth

Missing lower molars, causing upper molars to grow down below the bite plane

wearing of tooth surface due to abnormal chewing pattern

Missing teeth for a long time

We use T scan to record any abnormal occlusal patterns after replacing the missing teeth with dental implants. Single piece Bicortical implants and permanant teeth immediately are planned for this patient.

FAILED DENTAL IMPLANTS- HOW TO PROCEED

Failed Dental Implants-How to proceed?
Dental Implant Failures!!

Dental implant failures are a night mare to both patients and dental implantologist together.
The better term would be IMPLANT LOOSENING!!
Causes for dental implant failure
Its a multifactorial process..
The possible causes of dental implant loosening are
   1.  Lack of primary stability..that means how much stable the implant was during the placement
   2.  Over drilling
   3.  Over heating during implant osteotomy
   4.  High points in the teeth
   5.  Lateral interfernces during chewing
   6.  Patient biting hard food during inital healing period
   7.  Gross lack of oral hygiene by the patient
   8.  Parafunctional habits like bruximsm, night grinding etc.
   9. Poor bone quality in the area especially maxillary implants

After placement of BCS implants

After giving hybrid denture

Dental implant failure is not the end of the story.Various treatment options available to correct the situation,,
They are..
1.Removing the failed implant and replacing immediately with wider and longer implants..preferably with Basal Dental Implants ( either BOI or BCS)
   2. Removing the mobile implants and waiting for few weeks or months depending on the situation to allow bone formation. Then placing fresh implants .
3.Going for implant removal and treating with different treatment options like tooth supported bridge or dentures!!
