Dental implants for failed root canal treated teeth in chennai, India

              This is case report of our 32 year old male patient who approached us with a complaint of severe pain in upper front teeth since 1 month. On questioning about the previous dental history, patient gave a history of  2 root canal treated teeth done 8 years back, and tooth supported metal ceramic crown was given to the patient. The other root canal treated tooth was asymptomatic. Patient was not able to speak, that even the tongue pressure over the tooth while speaking and chewing was giving severe pain to the patient, also the patient was not able brush his teeth. Patient itself came to know that the root canal treated teeth was reinfected. then we clearly explained that reinfection in root canal treated teeth is a normal complication and that too it depends on aggressive nature of the bacteria, patient finally understood the current situation of his painful teeth and asked for a permanent solution that would improve his well being.

                               ON CLINICAL EXAMINATION

            The tooth was tender on percussion and palpation of surrounding tissues, adjacent tooth was asymptomatic and there was no tender on palpation and percussion.

ON RADIOGRAPHIC EXAMINATION

A periapical pathology indicating pus formation around the apex of the teeth was noticed and mild to moderate bone loss was noticed around the concerned teeth

TREATMENT PLAN

After complete clinical and radiographic analysis,, treatment plan was discussed with the patient, first extraction of painful root canal treated tooth followed by replacemnet with basal dental implants in a day and giving connected tooth supported PMMA crown on the same day of implant surgery.

TREATMENT PROCEDURE 

Before starting implant and tooth removal procedure, tooth supported metal ceramic crown was removed followed by extraction and implant placement procedure under local anesthesia. 

IMMEDIATE POSTOPERATIVE OPG 

Following the implant placement procedure, 3D digital scanning using imedit500 intra oral scanning was done and  tooth and implant  supported PMMA bridge was fixed on the same day of surgery

 A final outcome, in which the prosthesis is natural looking and mimicks tha adjacent teeth both shadewise and contourwise, and the patient was extremely happy with the final outcome '

Patient was fully satisfied with his appearnce and left back to banglore, and he was asked to report us after  4-6 months of healing period, for changing his provisional(temporary crowns) to permanent zirconium crowns.

Back teeth replacement with dental implants in chennai, India

This is a case of 32 year old male with a complaint of pain in his right lower back teeth since 10 days. Patient gave a history of root canal treatment done 7 days back in outside private clinic, since then the pain did not subside due to severe periapical infection. Over the time pain increased , patient was worried with his oral condition, and finally he approached us seeking for a solution, that could restore his well- being.

EXAMINATION OF THE PATIENT
              On intraoral examination, root canal treated tooth exhibited grade 1 mobility, surrounding gum tissues were red, also a root fragment was present on his lower left 1st molar area

                                           

     Full mouth X-ray (OPG and CBCT) was taken, there was an infection around the concerned root canal treated tooth, also there was a infected root fragments in the left lower 1st molar

                           
TREATMENT PLAN
    After complete clinical and radiographic examination, we suggested to extract his infected failed root canal treated tooth on right side and infected root fragments on left side and replacing it with basal dental implants under local anesthesia in a day
      Followed by fixing provisional crown on the same day
      After 4 - 6 months permanent zirconium crowns by digital scanning will be fixed on the same
 day when the patient returns

Solutions to the problem of atrophic alveolar bone begin with understanding why unloaded bone goes atrophic. Nobody can exactly depict why alveolar ridge goes atrophic after extraction of teeth. Efforts have been made to elucidate the scientific basis for the observation that the integrity of the skeletal tissue has something to do with mechanical usage. Frost has evolved the concept of the mechanostat (Frost, 1996) which he himself proposed as a new paradigm of understanding the mechanical-usage-dedicated message traffic routes between the skeletal tissue and the circumstances in which it is tuned. He argued the mechano-biologic negative feedback mechanisms would adjust skeletal architecture under the control of a subject’s mechanical usage.

Strain-derived fluid flow transduces the strain information to stimulate bone metabolism.



Skeletal tissue has its loading history by dynamic loading which is remembered as a microstrain.
If the strain is too small value, resorption dominates formation of bone tissue and the bone goes atrophic.

If the strain is adequate, resorption balances formation and the bone maintains its volume.

If the strain is adequately high value, formation at last dominates resorption and the bone goes hypertrophic.

If the strain value is high by the overloading outreaching the range of physiological limitation, the strain is accumulated in the bone tissue causing microdamage leading to fatigue failure.
Finally, if the strain value over the limit of yield strength the bone goes spontaneous fracture.

Due to these reasons immediate loading of dental implants will result in functional replacement of teeth and prevent any further bone loss.

Although a number of fractures is still today preferably treated by stabilizing the sites with gypsum, surgical procedures and adequate medical devices have been developed during the last three decades, these include:

In orthopaedic surgery (knee and hip replacement surgeries) it is taught that immediate loading(using) after plating or screwing will lead to good healing and bone remodelling. The same concept is follwed in immediate loading dental implants. 

Primary implant stability refers to the stability of a dental implant immediately after implantation. Its value is derived from a mechanical engraving  of the titanium screw implant in the patient's bone tissue. High initial stabilization may be an indication for immediate loading with prosthetic reconstruction. When the healing process is complete, the initial mechanical stability is fully replaced by biological stability. The most critical moment for implantation success is the moment very less initial stabilisation, pending sufficient bone reconstruction supporting long-term maintenance of the implant. Usually this occurs during the 3–4 weeks after implantation. If primary stability was not high enough following implantation, the implant's mobility is high and can cause failure.

Malocclusion is the result of the body trying to optimize its function in a dysfunctional environment. It can be associated with a number of problems, including crooked teeth, gum problems, the temporomandibular joint (TMJ), and jaw muscles. Teeth, fillings, and crowns may wear, break, or loosen, and teeth may be tender or ache. Receding gums can be exacerbated by a faulty bite. If the jaw is mispositioned, jaw muscles may have to work harder, which can lead to fatigue and or muscle spasms. Case 1:
Patient came with the complaint of increased spacing observed recently .

On checking clinically we elicited, interference in the wisdom tooth and premolars. patient gives history of missing teeth since 9 years. gives history of  neck and shoulder pain for a long time which is intermittent.

On intraoral muscle evaluation, patient had severe pain in the muscle attachments on the right side, where there is interference.
