Immediate dental implants in chennai, India

 A  37 year old male patient from trichy approached us with a complaint of difficulty in chewing foods due presence of pain in his decayed teeth. Past dental history revealed that he lost his upper molar partially 9 years back due to deep dental decay. Somehow he can have soft foods but he is having difficulty in chewing hard foods. So, patient wanted to replace his decayed teeth with dental implants and to have an efficient chewing as early as possible 

                     CLINICAL EXAMINATION 
          On clinical examination there was a decayed root fragments in upper and lower 1st molar
               
                RADIOGRAPHIC EXAMINATION 
3D and 2D radiographic examination revealed good quality of basal and cortical bone for implant placement 

TREATMENT PROCEDURE 
After complete clinical and radiographic examination, treatment plan was discussed with the patient to extract decayed molar and placing dental implants of sufficient height and width at appropriate sites under local anesthesia(STA painless anesthesia) and giving provisional crowns on the same day and to give permanent zirconium crowns after 4-6 months of healing 

DAY 1 - extraction of decayed molar and replacement with dental implants was done on the same day under computerised local anesthesia and impressions were recorded for fixing provisional crowns
On DAY 2 - provisional dental crowns were fixed and the patient was recalled after 4-6 months of healing period for fixing permanent zirconium crowns

3D digital impressions were recorded on the same day for fixing CAD CAM milled  zirconium crowns 



ZIRCONIUM CROWNS were fixed next day and patient started to chew his favorite foods

Front teeth replacement with dental implants at affordable cost in chennai, India

               A 39 year old male patient from chennai came to our clinic with a complaint of shaking lower front teeth since 1 year, because of which he was also having gaps in his front teeth. On questioning about his past dental history, patient told that he underwent implant procedure and the tooth next it was shaking and giving gap. Patient told that he is having bleeding in his gums while brushing and also having difficulty in chewing foods. The patient finally approached us seeking treatment for his shaking teeth and wants a painless treatment with fast recovery. Since it was a front teeth  and also he is a social speaker he stressed that he cannot be without teeth and asked us to fix the teeth on the same day or the next day of the treatment. 

               ON INTRAORAL EXAMINATION 

             Implants and the teeth fixed to it was good was stable, the teeth next to the implants were grade 1 mobile with gaps present between them. The surrounding gums were inflammed and bleeding on probing.
                    

3D AND 2D RADIOGRAPHIC EXAMINATION

Both 3D and 2D xrays shows good quality of cortical and basal bone for accurate placement of implants.


TREATMENT PLAN DISCUSSION

After complete clinical and radiographic examination, treatment plan was discussed with the patient to extract the 2 shaking teeth and placing 2 additional implants under local anesthesia(STA painless computerised anesthesia) and giving provisional teeth on the same day. And we informed patient that after 4-6 months of healing, these provisional crowns should be changed to permanent zirconium crowns.

                 TREATMENT PROCEDURE

On day 1, implant placement procedure was done under local anesthesia and impressions were taken for delivery of provisional crowns. And the provisional teeth were fixed on the same day. The patient is asked to report us after 4-6 months of healing period for changing his provisional crowns to permanent zirconium crowns

Finally our patient reported us after 4 months healing period to change his provisional teeth. Patient was happy with the provisional teeth but he was more eager to use zirconium crowns for chewing and came back to us.

Affordable dental implants for missing teeth in chennai, india

          This case report is about our male patient of 29 years from sydney who reported to us with a complaint of missing and few decayed teeth in his lower jaw. This was his first visit to dental treatment, and since childhood he was having fear of dental treatments because of pain and injections. Over years and years he was having difficulty in chewing foods because lower molars on both sides were in trouble and was giving discomfort and pain . Because of difficulty in chewing, he was having restriction in taking his favorite foods and can have only soft diets. He finally decided to undergo dental treatment for his missing and decayed molars, keeping his fear in dental treatments aside. Patients told his main concern was to have a painless dental treatment and particularly stressed that before leaving chennai, he should have his favorite food in chennai that is briyani which is a favorite food of all of us.
  
                     ON CLINICAL EXAMINATION 
     
                   There were missing lower first molar on left side and deep dental decay on both side  lower molars which was tender on percussion, calculus and tartar was also present.



                  RADIOGRAPHIC EXAMINATION

                   Deep dental decay with pus formation around the apex of  left side 2nd molar, right side 2nd molar and wisdom tooth was noticed and the bone quality around the missing teeth was of sufficient height and width for implant placement.



             TREATMENT PLAN DISCUSSION 

        After complete clinical and radiographic examination, treatment plan was discussed with patient to extract the decayed molars and replace it with dental implants of sufficient height and  width under local anesthesia (STA- painless anesthesia) followed by giving provisional crowns on the same day of implant surgery
    
OUR STEPWISE TREATMENT PROCDURE 

 DAY 1 - under local anesthesia(STA- painless anesthesia) extraction of decayed molars and dental implants of required number and of sufficient height and width was placed, followed by  digital scanning(digital impression) was done for crowns

Before leaving to sydney that is after 2 weeks of our dental treatment, patient reported to us for check up and told his chewing efficiency was very good, that he started to have his favorite food that is briyani because of which he came to Chennai, Tamil nadu.

Missing teeth replacemnt with dental implants in chennai, India

                     This is a case report of our 31 year old female patient who came with a complaint of few missing molars since 10 years and deep dental caries in her lower 1st molars. The reason for her missing teeth is that she underwent extraction of  her teeth because of multiple decays and she was not able to chew foods since the molars on both sides were missing. Patients major concern was to have nuts and groundnut cakes which were her favorite foods after 3-4 months of treatment and told that she stopped having her favorite nuts since 10 years. As an end she approached us seeking a permanent solution to improve her chewing both functionally as well as esthetically.

                      ON CLINICAL EXAMINATION

                            There was a missing upper molars and deep dental caries in her lower 1st molars which was very painful, generalised calculus was present.



ON RADIOGRAPHIC EXAMINATION 

Bone quality in edentulous site appears to be good for dental implant placement. And there was  a deep dental caries in lower 1st molars.


TREATMENT PLAN 

After complete clinical and radiographic examination, treatment plan was discussed with the patient to replace the edentulous site with 3 dental implants and root canal treatment for lower decayed 1st molars followed by giving provisional crowns in a couple of days.

TREATMENT PROCEDURE 

First, dental implants in the edentulous site and root canal treatment was done on day 1 under local anesthesia, digital scanning for crown fixing was done.

Day 2 - cad cam milled pmma crown was fixed for the implants and root canal treated teeth with perfect shade matching 


Patient finally regained her chewing and started enjoying her favorite foods the very next day if implant surgery without any pain throughout the procedure.
    
Finally the patient was asked to report us after 4-6 months of healing period  to change her provisional crowns to permanent zirconium crowns

THE UNDERSTANDING THAT  UNLOADED BONE CAUSE MORE BONE LOSS has brought an awareness towards replacing missing teeth and at the same time prevent bone loss.

Solutions to the problem of atrophic alveolar bone begin with understanding why unloaded bone goes atrophic. Nobody can exactly depict why alveolar ridge goes atrophic after extraction of teeth. Efforts have been made to elucidate the scientific basis for the observation that the integrity of the skeletal tissue has something to do with mechanical usage. Frost has evolved the concept of the mechanostat (Frost, 1996) which he himself proposed as a new paradigm of understanding the mechanical-usage-dedicated message traffic routes between the skeletal tissue and the circumstances in which it is tuned. He argued the mechano-biologic negative feedback mechanisms would adjust skeletal architecture under the control of a subject’s mechanical usage.


Strain-derived fluid flow transduces the strain information to stimulate bone metabolism.



Skeletal tissue has its loading history by dynamic loading which is remembered as a microstrain.
If the strain is too small value, resorption dominates formation of bone tissue and the bone goes atrophic.

If the strain is adequate, resorption balances formation and the bone maintains its volume.

If the strain is adequately high value, formation at last dominates resorption and the bone goes hypertrophic.

If the strain value is high by the overloading outreaching the range of physiological limitation, the strain is accumulated in the bone tissue causing microdamage leading to fatigue failure.
Finally, if the strain value over the limit of yield strength the bone goes spontaneous fracture.


Due to these reasons immediate loading of dental implants will result in functional replacement of teeth and prevent any further bone loss.

PRINCIPLES OF ORTHOPAEDIC BONE SURGERY AND A COMPARISON TO DENTAL IMPLANTOLOGY

Although a number of fractures is still today preferably treated by stabilizing the sites with gypsum, surgical procedures and adequate medical devices have been developed during the last three decades, these include:

1. Screws and plates
2. Intra - bony nails
3. Intra - bony blades
4. Basal implants



Dental Implants and the prosthetic construction of the same resemble the same. 

The intraoral situation is special and different from extra- oral applications
1. The mucosal penetration area around dental implants is under constant and vicious bacterial attack.
2. The saliva washes away bacteria as their excretion products and it stabilizes the pH.
3. Jaw bones are not at all prone to osteomyelitic activity

In orthopaedic surgery (knee and hip replacement surgeries) it is taught that immediate loading(using) after plating or screwing will lead to good healing and bone remodelling. The same concept is follwed in immediate loading dental implants. 




Primary implant stability refers to the stability of a dental implant immediately after implantation. Its value is derived from a mechanical engraving  of the titanium screw implant in the patient's bone tissue. High initial stabilization may be an indication for immediate loading with prosthetic reconstruction. When the healing process is complete, the initial mechanical stability is fully replaced by biological stability. The most critical moment for implantation success is the moment very less initial stabilisation, pending sufficient bone reconstruction supporting long-term maintenance of the implant. Usually this occurs during the 3–4 weeks after implantation. If primary stability was not high enough following implantation, the implant's mobility is high and can cause failure.
