Long term neck and shoulder pain.... Did you check the teeth yet???

Dentistry is a science that is largely affected by its materials and equipment . Lost and damaged teeth 

require replacement or rehabilitation . Thanks to the development of differing dental materials and

the past quarter century, edentulous patients can masticate as if they had their own natural teeth 

instead of wearing uncomfortable dentures.

Malocclusion is the result of the body trying to optimize its function in a dysfunctional environment. It can be associated with a number of problems, including crooked teeth, gum problems, the temporomandibular joint (TMJ), and jaw muscles. Teeth, fillings, and crowns may wear, break, or loosen, and teeth may be tender or ache. Receding gums can be exacerbated by a faulty bite. If the jaw is mispositioned, jaw muscles may have to work harder, which can lead to fatigue and or muscle spasms. This in turn can lead to headaches or migraines, eye or sinus pain, and pain in the neck, shoulder, or even back. Malocclusion can be a contributing factor to sleep disordered.

Case 1:
Patient came with the complaint of increased spacing observed recently .

On checking clinically we elicited, interference in the wisdom tooth and premolars. patient gives history of missing teeth since 9 years. gives history of  neck and shoulder pain for a long time which is intermittent.

On intraoral muscle evaluation, patient had severe pain in the muscle attachments on the right side, where there is interference.

DO'S AND DONT'S AFTER TOOTH REMOVAL

WHAT TO EXPECT AFTER TOOTH REMOVAL

In most cases, the recovery period lasts only a few days. Take painkillers as prescribed by your dentist or oral surgeon. The following tips will help speed your recovery.


1. Bite gently on the gauze pad for an hour after tooth removal. Call your dentist or oral surgeon if you still have bleeding 24 hours after your surgery.
2. While your mouth is numb, be careful not to bite the inside of your cheek or lip, or your tongue.
3. Do not lie flat. Raise your head with support of pillows while lying down.
4. Try using an ice pack on the outside of your cheek for the first 24 hours.
5. Relax for a day. Physical activity may increase bleeding.
6. Eat soft foods, and soup. Gradually add solid foods to your diet as healing progresses i.e after 5 days
7. Do not use a straw for the first few days. Sucking on a straw can loosen the blood clot and delay healing.

Do not smoke for at least 24 hours after your surgery. The sucking motion can loosen the clot and delay healing. Also, smoking decreases the blood supply and can bring germs and contaminants to the surgery area.
Avoid rubbing the area with your tongue or touching it with your fingers.
Continue to brush your teeth and tongue carefully.
Your dentist will remove the stitches after a few days, if there is any.

BASAL IMPLANT Zygoma implant If you have been told that there is less bone in your upper jaw for dental implants, do not worry. The best solution for cases with severe bone resorption in upper jaw is Basal implant. Basal implants are replacing zygomatic implants as the best solution for patients requiring dental implants for upper jaw. Such patients were going for sinus lift or bone grafts as per old techniques. In a single procedure we place Basal implant and place the permanent crown in 3 days. As we use single piece implants the procedure is flapless and done under local anaesthesia.

When the patient requires fixed teeth on dental implants in upper jaw, then the options are:

» Cantilever bridges: For example in All-on-4 or All-on-6 techniques, implants are placed in anterior regions only and as such 1st molar crowns are placed as cantilevers and 2nd molars are not replaced at all. Sometimes only 10 teeth are placed. Another disadvantage of cantilevers is that the permanent ceramic bridges are fixed after 3-6 months.

Basal implants are implants located even when bone is less in upper jaw. They allow the prosthetic rehabilitation on dental implants in upper jaw. The applied forces go to the pterygomaxillar buttress area allowing immediate loading of the implants.
